SUMMARY: I'm trying to make a user form that people can use to create a action item list. It just seems like way the user form links to the data table isn't dynamic enough for me to make this work.

Example of an output: SEE PICTURE #2 (this could be in PDF i suppose, but preferably i can just save a filter called "action item list" and then export to csv.)
attempt #1: rename the subject of some of the text box to "x" then filter to show "x" only this would get rid of some of the rectangles that are grouped into this user form. I still have an issue with the "pipe supply" text being on a separate line from the "action by" text.
attempt #2: try some crazy java script?
ALSO: It is able to number the coordination items in sequence. SEE PICTURE #3
