Yes, that is also an issue I have seen. Bluebeam used to be cutting edge, it has fallen off with minimal improvements over the last 5 years. We also want a less secure version of studio sessions where multiple users can edit each others markups. Asked for that 7-8 years ago…crickets.

You just gave me an idea. Maybe this will help some in the meantime? I just made a bullet list using the character mapping you just used, and then saved it as a tool. Now I have a simple bullet list avaliable at any time. :)
I shared the tool set (attached) with just that tool in it in case it is needed for anyone browsing these parts.

I second what @Shaya Birnbaum said. The list of cusom columns can be exported to be used in other profiles, but in order to keep the columns consistent across various documents and sessions of Revu, you'll need to save the Markups List layout through saving it, along with all the other Profile components as shown in Shaya's comment. I just want to warn you that saving the profile will save all Profile components to the Profile. So, if you've made changes to your tool chest, toolbars, panel access bars, navigation bar, or status bar, these things will be saved to the currently in-use Profile.

Re: Shortcut for a personalized markup
- place the custom markup tool in a pdf and press escape (I was only able to modify the existing ones: ex: callout, cloud, text, etc.). I wanted to use blue instead of red, see picture bellow.
- right click the markup and choose set as default.
- Click on Revu on the top ribbon and choose keyboard shortcuts.
- Click on the markup you want to reassign. and enter the letter or number under the shortcut box.
- If you don't want to reassign, copy the shortcut letter associated with that tool.
- go back to the pdf and enter the letter assigned to that markup and the custom markup will be placed in the document.
- press the escape key and you can close out of that document.
- Every time you want to place that markup tool, enter the letter or number assigned to that markup.

Re: How to print markups outside the document area
I don't think you can get that to work. I'm pretty sure you can only print what is in the confines of the page. It might be good to determine the space you need to be able to add all of those markups to a page, then create a blank page, screenshot the original document, copy each of the markups outside the original page area, paste them into the new document, and then print that.
The reason I don't think it's possible is because of what happens when you go to flatten those markups. They get erased outside the page boarder. The markups are still exportable, and the view can still be shown to others from Bluebeam, but I think you have to create a document that's large enough to contain everything in order to print everything.

The markups have to be on the sheet for them to Print. I see two options for you.
A: Export the markups (either to a spreadsheet or a PDF).
B: Change the page size, so that the markups on on the "Paper" but outside of the original drawing.
To Do this, click on "Document", "Page Settings", Select either all pages, or current page.
- Change the Media Size
- Remove "AUTO SCALE"
- Adjust the scale.
- You can also might also want to center the media by adjusting the offset "X and Y).
