duplicate form field names

When using Automatic Form Field Creation process it duplicates form field names on each page. This causes issues when the document is used in other places by duplicating the content that is entered in those fields across the pages.

If the fields could be a different name on each page it would be ideal.

Another way to help identify the duplicate form field names would be to enable the sorting of the Form Fields by name.
